[ CT THORAC SPINE : 25 Mar 2010 11:50 AM ]

Findings : There is approximately 29 degrees of dextro-convex scoliosiscentered at T10. There are 12 rib pairs. The 10th and 11th ribs on theleft are fused for a short segment posteriorly and then reseparate laterally. There is fusion of the lamina from T8 through T10 on theleft. There is a hemivertebra at T11 on the right. The lamina on theleft at T11 is formed while the hemi vertebral body  and the pedicle are not formed on the left. There is a butterfly vertebra at T10. The left pedicle at T10 is not completely formed. There is spina bifida occulta at T10, T11 and T12. The right lamina at T8 does not join withthe left lamina at T8 to form a normal spinous process. The lamina at L3-L4 are fused bilaterally. The spinal canal appears patent throughout.

Impression : Congenital spinal anomaly from T8 through T12 as detailed above whichis most notable for a hemivertebra at T11 on the right. Additionalspinal anomaly noted at L3-L4. Approximately 29 degrees ofdextro-convex scoliosis centered at T10.

[ MRI COMPLETE SPINE : 25 Mar 2010 01:08 PM ]

Findings : The cervical spine appears within normal limits. There is approximately 29 degrees of dextro-convex scoliosis centeredat T10. There are 12 rib pairs. The 10th and 11th ribs on the left are fused for a short segment posteriorly and then reseparate laterally. There is fusion of the lamina from T8 through T10 on the left. There is a hemivertebra at T11 on the right. The lamina on the left at T11 is formed while the hemi vertebral body  and the pedicle are notformed on the left.  There is a butterfly vertebra at T10. The left pedicle at T10 is not completely formed. There is spina bifida occulta at T10, T11 and T12. The right lamina at T8 does not join with the left lamina at T8 to form a normal spinous process. There is a syrinx that measures 1.5 cm in length at the level of T8 extending to thesuperior endplate of T9. There are 5 lumbar type vertebral bodies. The conus medullaris appears tethered at the level of S2 where there is a lipoma of the filumterminale and a larger lipoma within the canal at the level of S3major 1.3 cm in length. There is a spina bifida occulta defect beginning at the level of S1 extending inferiorly throughout thesacral segment. There appear to be 5 sacral segments ventrally and incompletely formed posterior sacrum. The coccyx may be oriented horizontally and the segmentation is uncertain. Overlying this regionin the dorsal subcutaneous tissues there are two areas with linear decreased signal that could represent vascularity, but could possibly represent a dermal sinus. The other possibility is that there has beenprior surgery in this region and this may represent a small amount scar formation. The lamina at L3-L4 are fused bilaterally. There is a near congenital fusion of the L3 and L4 vertebral bodies with a small disc space and narrowed waist.

IMPRESSION : Multiple congenital spinal anomalies as detailed abovewith a tethered cord and conus at the level of S2. Additionally there is a syrinx in the thoracic region. Dextro-convex scoliosis with a hemivertebra at T11. There is a possibility of 1 or 2 dermal sinuses in the sacral region versus scar formation from prior surgery.
